Hemodynamic instability (inotropes, ≥2 saline boluses) Hemodynamically significant PDA Severe hypoxia (unresolved) GI anatomical pathology or obstruction Metabolic acidosis (base deficit > -10) Unstable respiratory distress (worsening support needs) MAP below age-appropriate minimum Pressors required (excluding dopamine ≤5 mcg/kg/min)
